I know this is an old thread but fuel pump + filter would be my first thought too. Seen this happen before.

Just for info to all, my mondeo MkIII did the same no rev over 4000-4500 , no acceleration, engine knock and a strange engine noise.. I replaced mostly everything because of the knocking which mislead me alot.
I replaced Knock sensor, cam sensor, crank sensor, MAP sensor, ignition coil, plugs and cables, fuel pump, fuel filter and the timing chain.. Nothing helped at all ! The engine did precisely the same, then i took of the cat and the engine reved over 6500 like nothing.. Took out the core and it really looks normal but for some reason the air can't pass thru the cat.. Now the car finally runs again  ;)

Not seen that issue for a long time, I read about it somewhere and then at

work a few days later one of the lads had an issue where his Rover Tomcat

would not rev up, dropped the exhaust after work and the cat had collapsed

and blocked the exit pipe.
